Sports Physicals

IMPORTANT REMINDER
All students participating in any school sport will need a current sports physical, signed Athletic Policy and Concussion form on file in the LMS school office by Wednesday, August 1, 2018. Students trying out/participating in Softball, Baseball and Cross Country must have their information turned into the LMS office by June 15. Athletic Policy and Concussion Forms are part of your student's online registration. Students who do not have the required paperwork on file will not be eligible to try out or attend practice for any athletic activity. The LMS office hours are 830a-230p, Monday-Thursday for paperwork drop off. If you have any questions please call the LMS office at 815-933-2243.
