Welcome to the Herscher CUSD No. 2 Health Office

 

ATTENTION PARENTS OF KINDERGARTEN, 2ND, 6TH, AND 9TH GRADE: SCHOOL DENTAL EXAMINATIONS ARE DUE MAY 15TH, 2024. THE FORM MUST BE COMPLETED WITHIN 18 MONTHS PRIOR TO THE MAY 15TH DEADLINE. THE PROOF OF DENTAL EXAMINATION FORM CAN BE FOUND BELOW.

NEW SEIZURE SMART ACT: PA 101-0050: SEIZURE SMART SCHOOL ACT
Creates the Seizure Smart School Act. Requires the parent or guardian of a student with epilepsy who seeks assistance with epilepsy-related care in a school setting to submit a seizure action plan with the student's school. Effective January 1, 2020.​ Seizure Action Plan

 

Child Health Examination Forms

Students entering kindergarten (or first grade if not previously enrolled in kindergarten), sixth grade and ninth grade must provide proof of physical examination on a form approved by both the Illinois Department of Public Health and the Illinois State Board of Education. Students entering an Illinois school from out of state or out of country for the first time at any grade level must also provide the physical exam form. The exam must be completed within a year (365 days) prior to the first day of the current school year. State of Illinois Certificate of Child Health Examination

Transfer Students

Students who transfer from out of state are required to submit a health exam with immunizations within 30 days of starting school. If the required exam and immunization records are not submitted within 30 days of starting school, then the student is not allowed to attend school until the required documentation has been submitted. Physical examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history. Eye examination must be performed by a licensed optometrist within one year prior to the first day of school or a new vision exam from an Illinois optometrist/ophthalmologist is due October 15th of that school year.

Students who transfer from other Illinois Schools will need transfer of current Illinois school health records (physical exam and updated immunizations, dental exam, eye exam)

Dental Exam Form

Children entering Kindergarten, 2nd, 6th and 9th grades are required to submit a dental form. Dental exam's must be completed by a licensed dentist. Here is the link for the form. Exams are due May 15th of the current school year. Exams must have taken place within 18 months prior to the May 15th deadline. A reminder will be sent out to parents at least 60 days prior to the May 15th deadline. A waiver form may be submitted in place of a dental exam if a student shows undue burden or lack of access to a dentist.

 

Eye Exam Form

All students enrolling in Kindergarten and students enrolling in an Illinois school for the first time must submit an eye exam. The requirement does not apply to preschool students. Here is the link for the form. Exam's must be completed within one year prior to the first day of the school year. Eye exam must be submitted by October 15th. A waiver may be submitted in place of an eye exam if a student shows undue burden or lack of access to an optometrist of physician who performs eye exams.

Objections to Exams and Immunizations 

 MEDICAL OBJECTION

Medical exemption for a student by which a physician may exempt a child from a required immunization. A statement from the MD, DO, APN or PA stating immunity from a vaccine or that an immunization is medically contraindicated is acceptable. Should the condition of the child later permit authorization, the requirement will then have to be met. If an outbreak occurs in the school setting, the student must be excluded from school for 14 to 21 days per IDPH.

RELIGIOUS EXEMPTION

 Requires the Illinois Certificate of Religious Exemption Form from the State of Illinois  with a physician signed statement explaining the objections to physical exam, health screenings, and/or immunizations on religious grounds. If an outbreak occurs in the school setting, the student must be excluded from school for 14 to 21 days per IDPH. 

 

Medication Administration

Any over the counter medication (Cough Drops, Tums, Tylenol, etc.) and prescription medication require a doctors order. Students will not be given prescription and/or over-the-counter medication without a completed NEW Medication Authorization Form  signed by the parent and students physician. All medication MUST be brought to school in the original container and labeled with the student's name. A reminder will be sent out at the end of the school year to all parents whose child may have taken medication during the school year. At the end of the school year, any medication not picked up by the parent will be disposed of. All emergency medication (i.e. inhaler, Epi-Pen, diabetes supplies) will be sent home with the student at the end of the year.

 

Asthma medication and administration

Students with asthma have the option to self-carry their inhalers while in school, at school sponsored activity, before or after normal school activities. A Student Self-Administration Consent Form is required. Please attach a copy of the original prescription label to the consent form. A physician signature is not required, only the parent/guardian signature is required in the State of Illinois. Public Act 096-1460 .

Please note: Illinois Public Act 099-0843 requires schools to request from the parent or guardian of students with asthma an Asthma Action Plan (AAP) The Asthma Action Plan can be filled out by the student's primary care provider and submitted to the school nurse.

 

Epi-Pen

Students with severe allergies and asthma may require an Epi-Pen emergency auto-injector for anaphylaxis. Students and parents have the option to self-carry their Epi-Pen while in school, while at school sponsored activities, or before or after normal school activities. Students may also keep their Epi-Pen in the main health office. Written authorization is required from the student's parent/guardian and student's physician. Please fill out the Illinois Allergy Emergency Action Plan  or a Medication Authorization Form if your child has a severe allergy and requires emergency medication. Public Act 096-1460 .

 

 PUBLIC ACT 096-1460

A school, whether public or nonpublic, must permit the self-administration of medication by a pupil with asthma or the use of an epinephrine auto-injector by the pupil, provided that:

  1. The parents or guardians of the pupil provide to the school written authorization from the parents of guardians for the self-administration of medication or for the use of an epinephrine auto-injector, written authorization from the pupil's physician, physician assistant, or advanced practice registered nurse; and
  2. The parents or guardians of the pupil provide to the school the prescription label, which must contain the name of the medication, the prescribed dosage, and the time at which or circumstances under which the medication is to be administered, or for use of an epinephrine auto-injector, a written statement from the pupil's physician, physician assistant, or advanced practice registered nurse. The information provided shall be kept on file in the office of the school nurse or, in the absence of a school nurse, the school administrator.

Vision & Hearing Screenings

Screenings start in the fall and will continue throughout the school year

Vision Screenings mandated per IDPH and ISBE: Pre-K, K, 2, 8 and all children with IEP's in the district

Hearing Screenings mandated per IDPH and ISBE: Pre-K, K, 1, 2, 3 and all children with IEP's in the district

If the parent/guardian does not want their student screened please put in writing and send to Kimberly Henderson, school nurse.  (notice per IDPH request.)

Vision screening is not a substitute for a complete eye and vision examination by an eye doctor. Your child is not required to undergo the vision screening if an optometrist or ophthalmologist has completed and signed a report indicating that an eye examination has been administered within the previous 12 months.
