LMS Athletics Reminder
All 6th, 7th and 8th grade students planning to participate in school athletics will need the following on file in the school office prior to participation in tryouts and/or practice:
-Current Athletic Physical
-Birth Certificate
-Verification of Health Insurance
-Athletic & Activity Policy
-Concussion Protocol
-School Fees paid or Monthly Payment Plan
(Health Insurance/Athletic Policy and Concussion can be taken care of during the registration process)
