OAK Orthopedics is offering FREE Sports Physicals on Saturday, August 10 from 9a-11a at 6712 N. Convent Street in Bourbonnais. This is for students in grades 5-12.
You will need:
2024 IESA/IHSA Completed Physical Form/parent must sign form.
Students should wear tank top and shorts.
